Form 1095-C Provided by Request Only

The West Linn-Wilsonville School District will provide Form 1095-C to full-time employees (and covered individuals) in paper form by request only.

If you would like to receive a paper copy of your Form 1095-C, please submit your request using one of the options below.

Be sure to include your full legal name and either your email address or mailing address where the form should be sent.

Voluntary Life Insurance helps protect and secure your family’s future in the event of death.


West Linn-Wilsonville School District offers the opportunity for you to purchase additional coverage for yourself and for your family members.  Your election may be made in $10,000 increments, not to exceed 5 times your salary or $300,000, whichever is less.


For Voluntary Life on yourself, your spouse, and your children, you are only eligible for Guarantee Issue amounts if you are a NEWLY ELIGIBLE employee of West Linn-Wilsonville School District.


Otherwise, coverage elected will be subject to completion and approval of the Statement of Insurability form. Email the form to Membership@USAbleLife.com within seven (7) days of submit your enrollment.

Supplemental Retirement Savings Programs


The West Linn-Wilsonville School District offers two excellent programs through which you may contribute a portion of your current income into supplemental retirement savings accounts.  The programs are the WLWV 403(b) Tax Sheltered Annuity Program and the WLWV 457(b) Deferred Compensation Program.

Domestic Partner Coverage
 

Domestic Partner Affidavit

 

The West Linn-Wilsonville school district allows coverage for unregistered and registered domestic partners. The premium for coverage of a domestic partner will NOT be different from the premium for coverage of a spouse. You are required to fill out the Domestic Partner Affidavit within seven (7) days of completing your enrollment.

Please note: The IRS does have tax implications that will be charged to your monthly paycheck.
