On The Job Injury
In the event of an on-the job injury, please complete an injury report (Declaración de incidente) at your work site immediately following an injury. If you need to be seen by a doctor, please contact Human Resources at 503-673-7004. Unless it is an immediate emergency, there is paperwork you will need to complete before receiving treatment. Employees can also download instructions for filling out an injury report.
