Administering Medications to Students

Oregon State Law requires the following procedures to be used when a physician has ordered prescription medicine to be take by a student at school, or when over-the-counter medicine is to be taken. For the safety of all children, please follow this procedure.
- ALL medication need to be sent to school in their original container, properly labeled with the student's name, time and amount of dosage, the name of the drug, and the name of the prescribing physician. DO NOT SEND ANY KIND OF MEDICINE IN A PLASTIC BAGGIE!
- The medication must be accompanied by a request, filled out and signed by a parent/guardian, for school staff to administer the medication. This form is available from the school office, and online using the link below.
