• Date of Birth
• Street Address
• Phone
• Year of graduation
• Institutions you would like your transcript sent to (name/address)

By submitting a request, you are authorizing West Linn High School to release your transcript to the institutions listed. Transcripts are typically sent within 48 hours of request receipt.
